Home/hospital instruction (i.e., homebound) provides students with extenuating short-term medical circumstances the opportunity to continue with their studies. KRS 159.030, Section (2) provides schools and districts the opportunity for students deemed unable to attend school due to a medical and/or mental chronic condition that limits the student's ability.
As required by the statute, a licensed physician, advanced registered nurse practitioner, psychologist, psychiatrist, chiropractor or public health officer must complete the Home/Hospital Application and submit the completed document to the District Office. If a student will likely be absent from school greater than 6 months, two (2) signed statements (applications) from two different local health personnel must be submitted.
A student approved for home/hospital instruction will be served by a certified teacher employed by Logan County Schools. The teacher will consult with the student's teachers at their school, collect assignments, and meet with the student at their home for a minimum of 2 hours per week. As required by statute, the students parent and/or guardian must be present during the visit by the homebound teacher.
If a student anticipates being absent from school greater than 5 consecutive days, it is recommended the student and parent/guardian submit the application at the right to their local health care professional and submit to the District Office by fax (270.726.8892).
