Student Registration

We are so happy to welcome you to our Logan County Schools family!

The first step in the registration process is to click on the registration form below.  (also found at this link)

Next, you will need to gather the following documents and submit them to the school counselor or office staff:  

• A legal birth certificate or other reliable proof student age
• Proof of residency (example:  utility bill, rental agreement, etc.)
• Proof of a preventative health care exam (school physical) conducted within six months of initial entry into school.  Another exam is required before entry into 6th grade.  Link to KY school physical form
• Proof of an eye exam between ages 3-6 by an optometrist or ophthalmologist, documented on the KY form.  Due by Jan 1 of first year child is enrolled.  Link to KY Eye Exam Form
• Proof of a dental screening documented on the KY Dental Screening Form.  Due by Jan 1 of the first year child is enrolled.  Link to KY Dental Screening Form
• Current immunization certificate.  Due within 2 weeks of child's enrollment.  Link to KY Immunization Form
